From the beginning of April employers (me!) are required to send accurate details every month to the tax man on who I am paying and how much!
As part of this I need to send all employees full names including middle names.
I do not have all this info!

At the same time I could also do with having your date of birth confirming - as I'm not convinced I have all these 100% accurate (Maz recently pointed out her DOB was down as 1st Jan 2000?!)

I have tasked Hemehra with extracting this info from you!
So, if you don't know your full name and date of birth (!!) - find out soon and let her know - or she'll be chasing you....
